The form below has been created so the Local Executive Committee has an up-to-date list of contact information for it's members.  Even though the employer has all your same contact information, they will not share with the union due to protection of privacy.

When updating information with the Employer (ie: WIN, etc), please remember to update your address and contact information here as well.
